Some of the most serious incidents in construction have started with those six words.

A worker skips a safety check because they are in a hurry. Someone decides not to wear a piece of PPE for a "quick task." A minor hazard is ignored because "nothing has happened before."

Most workplace incidents are not caused by a single major failure. They are often the result of small shortcuts that gradually become accepted as normal.
