1997 International box truck, all keys lost.
Picked the door open, looked up reference (to which there really is none when its O'dark:30 and a balmy 19°F) and decided I should just pull the ignition and bring it back to the shop.

Fire up the heater, grab a cup of coffee and do the work...

5 mins later and six blocks away from when I left that job my foot slips off the brake and I rear-ended an old Toyota Tacoma pickup at a stop light at about 3 mph. There was damage so it was called in.
The responding officer was at the scene I just left. As he was approaching us (the other driver and I) he said "So we meet again... No dead people this time I'm guessing?"

The look on the other driver's face.....

As Mrs Homeowner exclaimed "My husband... *what* was he thinking??"

Filled and caulked his attempted placement using the wood plug from the top, correctly placed hole, and installed a door wrap cover plate.
