Skimmer loose from wall causing a leak, I assume it was simply not fully installed during the build, no gaskets, gapped from the wall and concrete was poured and finished.

Second photo is a crack in the male adapter installed into the return fitting causing a small leak, cracks like these tend to get worse over time until separated completely.
