This is how I paint stairways. To work smart, I cut in twice at the top before I begin rolling. That way, I only have to set up the ladder once. Of course, I use an extension pole to reach the top.
FYI, not only do they make 18” roller covers, but they make 14” as well. Both provide excellent results.
Purdy makes an awesome roller cover called “Altitude” they are microfiber naps and offer great results.
