Businesses believe their IT is more secure than it really is

A new study reveals a structural cyber security problem in organisations, with a widening gap between assumed security and proven resilience.

Organisations are investing heavily in cyber security, yet many are still judging success by the wrong measures. That is the central finding of a new global survey from cyber security company Horizon3.ai. The report is based on a survey of 750 IT security professionals across Europe and the United States, including both senior security leaders and frontline practitioners. It was commissioned to examine the gap between executive confidence and operational reality.

The findings suggest many organisations continue to treat busy security programmes as evidence of security. Assets are scanned, alerts are generated, patches are deployed and dashboards are updated. Yet these activities do not always confirm whether exploitable weaknesses have been removed or whether defences would withstand real-world attacker behaviour.

High confidence but limited validation
The report called The State of Assumed Security 2026 suggests that confidence in existing controls is high among senior decision-makers. According to the survey, 93% of CISOs say they could demonstrate that their organisation had taken reasonable, validated steps to prevent a breach. Meanwhile, 97% are confident their endpoint protection would detect lateral movement or privilege escalation, while 96% believe their Security Operations Centre (SOC) could identify an attacker operating inside the environment.

However, day-to-day operational practice tells a different story. Only 30% of CISOs say their organisations patch vulnerabilities and then test to confirm that risk has been properly remediated. Nearly half patch systems and simply rerun a vulnerability scanner instead. Just 12% report validating the effectiveness of their Endpoint Detection and Response (EDR) tools within the past three months, while only 26% use red team exercises or pe*******on testing to assess the detection capability of their SOC. Among frontline practitioners, one third assume scanner findings are accurate without further testing, while 17% do not validate findings at all.

Understanding transparent architecture in access control systems

Typically, readers handle encryption — they decrypt credentials locally and send access decisions to controllers. But a different model has entered the conversation, at least in Europe: transparent architecture. Here, Steven Commander who works with HID Global explains how industry stakeholders will need to understand how transparent architecture works in readiness for the new and evolving European standards.

First, it is important to define what Transparent Architecture actually is. “Transparent architecture” sounds like it could mean many things. Transparent to whom? And about what?

According to Commander, the term describes an approach where readers become “transparent” to cryptographic operations. Instead of handling encryption themselves, readers capture credential data and pass it to more centrally located controllers to handle the cryptographic work.

Transparent Architecture increasingly prevalent in Europe
“This architectural shift is driven by regulatory frameworks, particularly in high-security environments,” says Commander. France’s ANSSI Certification de Sécurité de Premier Niveau (CSPN) certification requires transparent architecture for certain facilities. Government buildings, critical infrastructure and operators of vital importance must demonstrate that cryptographic operations occur in physically secured locations, not at exposed perimeter readers.

The approach is spreading. Similar frameworks are under consideration in Germany and the Nordic countries. The concern is consistent across these discussions: readers mounted on exterior walls, in parking structures or at building perimeters are more vulnerable to physical tampering than equipment housed in controlled server rooms.

To be clear, transparent architecture isn’t replacing traditional access control universally. Most commercial buildings, corporate offices, educational institutions and healthcare facilities will continue using reader-level encryption successfully. Transparent architecture addresses specific regulatory requirements for specific environments.

Access control must extend beyond the building

For many organisations, access control is still centred on the building. But operations and the majority of physical risk often are not. Jeroen van Os, Commercial Lead – Global Technology Partnerships at Nedap is urging companies to think more deeply about their access control systems to avoid risking gaps in their corporate security structure.

In the past, access control was designed around fixed locations, users, and credentials. But that model doesn’t work when assets are distributed.

Even if sites are visited infrequently, they still need to be protected. Especially as remote, often unmanned, locations are being targeted more frequently. And access needs to be easy for those authorised.

Without the right setup, issues can include technicians having to collect keys or be escorted onto site. Keys can be lost, shared or copied. Access isn’t time-bound. Sites can be left unsecured. And visibility is limited, making it difficult to track who accessed what, when, and why.

The impact can be significant
Focusing only on buildings for access management leaves gaps that, however small, can have a big impact. This includes:

Increased security risks: Unauthorised access becomes more likely when remote sites lack proper protection.
Operational inefficiencies: Time can be wasted and costs incurred in a range of ways, from credentials being issued manually to delays getting started on site.
Compliance challenges: Regulatory demands are increasing. NIS2, for example, requires physical access logs for all locations, not just connected sites.
Reporting is difficult: If there’s no record of who accessed what or when, audits are incomplete and investigations lack traceability.
