This usually happens at the position where there is toilet at the upper floor of a suspended concrete floor, this could be as a result of
1. weak/porous concrete work
2. Leakages from service pipes
3. Absence of waterproofing materials in the mortar used for laying floor and wall tiles in the toilet above
