This split counter plug was never split and the breaker didn't trip and worked for years.
I have seen the like twice now and don't know for sure how it happens. Both times I found it when I turned off a breaker and it tripped when turned back on.

I often had this happen to me when I was first trying finishing because I was using cheap tools. 'Use the proper tool for the proper job' isn't just something people say for not reason.
The screwdriver used to tighten this screw was too small. The tool marks show as much. A properly sized flat screwdriver not only matches the length of the slot, but also leaves no gaps on either side to move about. I find the larger flat in the Klein 6-in1 or 10-in-1 works perfectly for this.

Fried ballast anyone?
This cooked ballast was the cause of a fault that tripped a lighting circuit. In tracking down the fault, we found other issues, like broken bases and interesting mounting techniques for the ballasts in most of the fixtures.
There is no separate compartment for the wiring, so a lot of the wiring to the ballasts were run right over the lamp, cracking the insulation of most of the wires.

I noticed a railing was secured to only drywall at one end and the drywall was disintegrating.
I offered to secure it for a client and now it is solid, if not a work of art.

It is not okay to terminate a cable without a connector.
The person who installed this cable was also quite rough with the wires and damaged the insulation.
To top it off, the connections were all loose, except the bare bond wire.

Fuses aren't good forever.
I have seen several of these old NRN100 fuses fail recently, apparently without an overcurrent condition. This could be the fuse link material becoming brittle with age, repeatedly being pulled out, solder joints cracking or corroding.
These older fuses will be replaced as a group to prevent more untimely failures due to age.
The one fuse barely contained the arc from the fuse failing. Selecting a fuse with the proper interrupting rating is importent too, lest the fuse not be able to contain the energy of the arc.

A melted or burnt receptacle is what eventually happens when the 'stab-in' connections in the back of the receptacle are used instead of wire nuts (marrettes) to join the wires together.
The 2018 Canadian Electrical Code rule 4-022 1) d) says wiring must be installed in such a manner that any identified conductor can be disconnected without disconnecting any other identified conductor.
This rule means that the identified conductor (common or neutral) connection must be made independent of the device it is connected to. The most common way I've seen to do this is 'pigtailing', or connecting the wires together with a jumper going to the receptacle.
I've seen enough 'stab-in' connections fail that I do not use them, but use the screw connections instead for a better connection.
